A Zepbound tracker for pens and for vials
Zepbound is tirzepatide for weight management and sleep apnoea. Half the people on it now use single-dose vials, which means a syringe and some arithmetic. The app does both.
Zepbound is Eli Lilly's tirzepatide for chronic weight management, and since late 2024 for obstructive sleep apnoea in adults with obesity. It comes as a weekly pen and, through LillyDirect, as single-dose vials at self-pay prices. The vial route is cheaper and needs a syringe, so the tracker has to handle units as well as milligrams.
The schedule
| Period | Dose | Notes |
|---|---|---|
| Weeks 1 to 4 | 2.5 mg once weekly | Starter dose, not meant to work yet |
| Weeks 5 to 8 | 5 mg once weekly | |
| Each step after | +2.5 mg after at least 4 weeks | 7.5, 10, 12.5 mg |
| Maximum | 15 mg once weekly | Maintenance is 5, 10 or 15 mg |
In SURMOUNT-1, people on 15 mg lost about 21 percent of body weight over 72 weeks; on 5 mg, about 15 percent. The dose that is right is the one that moves weight with side effects you can live with, and plenty of people hold at 5 or 7.5 mg.
Vials
A Zepbound vial holds one dose in 0.5 ml. Draw the whole vial into the syringe; there is no reconstitution because it arrives in solution. Where people get confused is a compounded tirzepatide vial, which holds several doses and sometimes arrives as a powder. For those, the app's reconstitution calculator turns milligrams into units on the syringe, and the vial row counts doses down.
Missed a dose
Within 4 days, take it and keep the schedule. Past 4 days, skip. Never two doses within 3 days. Longer gaps are on the missed dose page.
Setting it up in Peptide AI
- Add Tirzepatide from the compound library. The unit and the weekly schedule are already set.
- Enter the titration your prescriber gave you. The app shows which week of which step you are on and when the next step is due.
- Add the pen or vial, with doses remaining and the expiry date. A refill reminder fires at three doses left.
- For a vial, log it as a vial with one dose. For a compounded multi-dose vial, enter the concentration and the calculator gives the units.
- Log each dose when you take it, with the site. Notes for side effects go on the dose.
The weight line, and what sits next to it
The free tier keeps the dose log, protein and training. Premium reads Oura, WHOOP, Garmin, Apple Health or Health Connect, so weight, sleep, HRV and resting heart rate sit on the same weeks as the dose steps. For sleep apnoea, the sleep line from the watch is the one to watch; the drug's effect on apnoea in the SURMOUNT-OSA trials tracked weight loss, and both show up there. Remi, on Premium, answers "what changed the month my weight stalled" from the log.
Logging on Peptide AI is free, and that includes GLP-1s. The dose log, the titration schedule, vials with doses remaining, injection sites with a rotation reminder, nutrition, supplements and training are all on the free tier with no card. Premium adds bloodwork, Oura, WHOOP, Garmin, Apple Health and Health Connect on the same timeline, the body scanner and Remi, the coach that answers from your own log. The pricing page has the three plans.
Questions people ask
Is there a free Zepbound tracker?
Yes. Peptide AI logs tirzepatide pens and vials, sites, titration and doses remaining free on iOS and Android. Weight and sleep from a wearable, and bloodwork, are Premium.
How do I track Zepbound vials?
Log the vial with one dose and the expiry. A Zepbound vial is drawn in full with no reconstitution. For a compounded multi-dose vial, the app’s calculator converts milligrams to syringe units.
What is the Zepbound dosing schedule?
2.5 mg once weekly for 4 weeks, then 5 mg, then 2.5 mg steps after at least 4 weeks each, up to 15 mg. 5, 10 and 15 mg are maintenance doses.
