A Mounjaro tracker with the titration ladder built in
Mounjaro is tirzepatide labelled for type 2 diabetes. The log that matters is the dose step, the shot day, and the HbA1c that comes back three months later.
Mounjaro is Eli Lilly's tirzepatide, approved for type 2 diabetes. It is a once-weekly injection on a 2.5 to 15 mg ladder, and for most people on it the point is blood sugar first and weight second. That changes what is worth logging: the dose and the shot day, as on any GLP-1, but also fasting glucose, HbA1c every three months, and anything else you take that lowers blood sugar.
The schedule
| Period | Dose | Notes |
|---|---|---|
| Weeks 1 to 4 | 2.5 mg once weekly | Starter dose, not meant to work yet |
| Weeks 5 to 8 | 5 mg once weekly | |
| Each step after | +2.5 mg after at least 4 weeks | 7.5, 10, 12.5 mg |
| Maximum | 15 mg once weekly | Maintenance is 5, 10 or 15 mg |
The label calls 5, 10 and 15 mg maintenance doses. Many people hold at 5 or 7.5 mg if blood sugar is where it should be. Steps are at least four weeks apart and often longer.
Missed a dose
Take it within 4 days (96 hours) of the missed day and keep the usual schedule. Past 4 days, skip it. Never two doses within 3 days of each other. The missed dose page covers longer gaps.
Setting it up in Peptide AI
- Add Tirzepatide from the compound library. The unit and the weekly schedule are already set.
- Enter the titration your prescriber gave you. The app shows which week of which step you are on and when the next step is due.
- Add the pen or vial, with doses remaining and the expiry date. A refill reminder fires at three doses left.
- If you also take insulin or a sulfonylurea, log them too. The low blood sugar warning on the label is about that combination, and the prescriber will want to see both lines.
- Log each dose when you take it, with the site. Notes for side effects go on the dose.
What to log beyond the dose
- Fasting glucose, if you test. Manual biomarker entry is free.
- HbA1c every three months. On Premium, a lab PDF is parsed into 36 biomarkers and the HbA1c line sits against the dose steps, which is the chart the review is about.
- Weight, weekly. From Oura, WHOOP, Garmin, Apple Health or Health Connect on Premium, or typed in.
- Side effects as notes on the dose: the day after the shot, the severity.
- Protein, daily, because appetite will fall and muscle should not.
Mounjaro and Zepbound
Same molecule, same pen, same doses. Zepbound is the weight and sleep apnoea label. If you are prescribed one and dispensed the other, the log does not change. The four-way comparison explains the labels.
Logging on Peptide AI is free, and that includes GLP-1s. The dose log, the titration schedule, vials with doses remaining, injection sites with a rotation reminder, nutrition, supplements and training are all on the free tier with no card. Premium adds bloodwork, Oura, WHOOP, Garmin, Apple Health and Health Connect on the same timeline, the body scanner and Remi, the coach that answers from your own log. The pricing page has the three plans.
