A Wegovy tracker for the five-step ladder
Wegovy is semaglutide for weight management. Sixteen weeks of titration, a 2.4 mg maintenance dose, and a missed dose rule that depends on the clock.
Wegovy is Novo Nordisk's semaglutide for chronic weight management, cardiovascular risk reduction in people with heart disease, and, since 2025, MASH. It is the drug behind the STEP trials, where people lost about 15 percent of body weight over 68 weeks at 2.4 mg. The ladder is five steps over sixteen weeks, and the single-dose pen means one pen is one shot.
The schedule
| Period | Dose | Notes |
|---|---|---|
| Weeks 1 to 4 | 0.25 mg once weekly | Starter dose, not meant to work yet |
| Weeks 5 to 8 | 0.5 mg once weekly | |
| Weeks 9 to 12 | 1 mg once weekly | |
| Weeks 13 to 16 | 1.7 mg once weekly | |
| Week 17 on | 2.4 mg once weekly | Maintenance dose |
If 2.4 mg is not tolerated, the label allows 1.7 mg as a maintenance dose. Any step can be held longer than four weeks, and prescribers often do.
Missed a dose
Wegovy's rule is about the next dose rather than the last one. If the next scheduled dose is more than 48 hours away, take the missed dose now and keep the schedule. If it is less than 48 hours away, skip. After more than two missed weeks, the label allows resuming as prescribed or restarting at a lower dose; that is a call to the prescriber. The missed dose page has every rule.
Setting it up in Peptide AI
- Add Semaglutide from the compound library. The unit and the weekly schedule are already set.
- Enter the titration your prescriber gave you. The app shows which week of which step you are on and when the next step is due.
- Add the pen or vial, with doses remaining and the expiry date. A refill reminder fires at three doses left.
- Each Wegovy pen is one dose, so log a box of four as four doses remaining and the app counts down to the refill.
- Log each dose when you take it, with the site. Notes for side effects go on the dose.
What to log for weight
- Weight, weekly, same day and conditions. From Oura, WHOOP, Garmin, Apple Health or Health Connect on Premium, or typed in.
- Protein, daily. The free food log shows it against your target. On a GLP-1 this matters more than calories; the muscle page explains why.
- Side effects as notes on the dose. Nausea is the most common reason people stop Wegovy and the easiest to manage with a dated log.
- Resting heart rate, from the watch. The label notes a small increase, and the wearable line makes it visible without worry.
- Mood. The label asks prescribers to monitor it. A note is enough.
Wegovy and Ozempic
Same molecule. Ozempic tops out at 2 mg and uses a multi-dose pen. If you were switched between them for coverage reasons, the log continues; only the pen type changes.
Logging on Peptide AI is free, and that includes GLP-1s. The dose log, the titration schedule, vials with doses remaining, injection sites with a rotation reminder, nutrition, supplements and training are all on the free tier with no card. Premium adds bloodwork, Oura, WHOOP, Garmin, Apple Health and Health Connect on the same timeline, the body scanner and Remi, the coach that answers from your own log. The pricing page has the three plans.
Questions people ask
Is there a free Wegovy tracker?
Yes. Peptide AI logs semaglutide doses, pens, sites and the five-step titration free on iOS and Android. Weight from a wearable and bloodwork are Premium.
What is the Wegovy dosing schedule?
0.25 mg weekly for 4 weeks, then 0.5, 1 and 1.7 mg for 4 weeks each, then 2.4 mg as the maintenance dose. 1.7 mg is the label’s fallback maintenance dose.
I missed my Wegovy dose. What do I do?
If your next scheduled dose is more than 48 hours away, take the missed one now. If it is less than 48 hours away, skip it. After more than two missed weeks, ask your prescriber whether to restart lower.
