You missed a dose. Here is what the label says to do.
The rule depends on the drug and on how late you are. Three windows cover the four big injectables. Then one question for your prescriber.
This is the page people search at 11pm. The short version is in the table. The rest explains why the windows differ and what to do after a long gap. None of it replaces a call to your prescriber, and the label says so too.
| Drug | Take the missed dose if | Otherwise |
|---|---|---|
| Wegovy (semaglutide) | The next scheduled dose is more than 48 hours away | Skip it, take the next one on the usual day |
| Ozempic (semaglutide) | It is within 5 days of the missed day | Skip it, take the next one on the usual day |
| Mounjaro and Zepbound (tirzepatide) | It is within 4 days (96 hours) of the missed day | Skip it, take the next one on the usual day |
| Wegovy pill (oral semaglutide) | Never double up | Skip it, take the next tablet the following morning |
| Foundayo (orforglipron) | Never double up | Skip it, take the next tablet the next day |
Why the windows are different
Semaglutide has a half-life of about a week. Tirzepatide's is about five days. A weekly injection keeps the drug at a steady level, and a few days late barely dents it, which is why the labels allow a late dose rather than a lost one. Past the window you are close enough to the next scheduled shot that taking both would stack them. Two doses within a couple of days is the thing every label tells you not to do.
Missed two or more in a row
This is where it stops being a rule and becomes a conversation. After two missed weeks of Wegovy, the label says to resume as prescribed or, if needed, restart at a lower dose. The reasoning is simple: the side effects you worked through in month one were your gut adapting to the drug. Two or three weeks without it and some of that adaptation is gone. Jumping straight back to 2.4 mg or 15 mg after a three week gap is how people end up with the worst nausea of the whole course. Many prescribers drop back a step and climb again.
Changing your injection day
You can move the day. The labels ask for at least 48 hours (Wegovy) or 3 days (tirzepatide) between the last dose and the new day's dose. Pick the new day, log it, and keep it.
What to do right now
- Work out when the last dose actually was. If you logged it, this takes one tap. If not, the pen's dose counter and your photos are the next best thing.
- Count the days to the next scheduled dose and find your row in the table.
- If you are inside the window, take it and keep the original schedule.
- If you are outside it, skip, and tell your prescriber at the next contact. Two or more missed, call them before the next shot.
Why this is the strongest case for a log
Every rule above starts with "how long has it been." A dose log with the time on it answers that in a second. Peptide AI also reminds you on the day, flags when you are past the window, and shows the gap on the timeline so the next review has the full story. That is on the free tier.
Questions people ask
I missed my Wegovy dose by 3 days. Should I take it?
If your next scheduled dose is more than 48 hours away, the label says take the missed dose now and keep your usual day. If the next dose is less than 48 hours away, skip the missed one.
I missed my Mounjaro or Zepbound dose by 5 days. What now?
Five days is past the 4 day (96 hour) window. Skip it and take the next dose on the usual day. Do not take two doses within 3 days of each other.
Do I have to restart at a lower dose after missing two weeks?
Not always, but it is on the table. The Wegovy label allows either resuming as prescribed or restarting at a lower dose after more than 2 missed weeks. Ask your prescriber, and mention how long the gap was.
Can I take two GLP-1 doses to catch up?
No. Every label says not to take an extra dose to make up for a missed one. The drug is still in your system for weeks, and doubling it is how the worst side effects happen.
