An Ozempic tracker that counts the pen as well as the dose
Ozempic is semaglutide labelled for type 2 diabetes, dosed from a multi-dose pen. The log has to know which pen, how many doses are left in it, and when the 5 day window closes.
Ozempic is Novo Nordisk's semaglutide for type 2 diabetes, with later indications for cardiovascular risk and chronic kidney disease in diabetes. It is also, off-label, one of the most prescribed weight loss drugs in the world. The pen holds four doses at the labelled strength, and because the 0.25 mg starter and the 0.5 mg dose come from the same pen, the first month's pen lasts a different number of weeks from the rest.
The schedule
| Period | Dose | Notes |
|---|---|---|
| Weeks 1 to 4 | 0.25 mg once weekly | Starter dose |
| Weeks 5 to 8 | 0.5 mg once weekly | |
| After that, if needed | 1 mg, then 2 mg once weekly | Each step after at least 4 weeks |
The 0.25 mg dose is for tolerance and is not meant to control blood sugar. Most people settle at 0.5 or 1 mg. The 2 mg dose is for those who need more glucose control after at least four weeks on 1 mg.
Missed a dose
Take it within 5 days of the missed day and keep the usual schedule. Past 5 days, skip it. Do not take two doses within 2 days. Changing your injection day is fine if at least 2 days have passed since the last dose. Longer gaps are on the missed dose page.
Setting it up in Peptide AI
- Add Semaglutide from the compound library. The unit and the weekly schedule are already set.
- Enter the titration your prescriber gave you. The app shows which week of which step you are on and when the next step is due.
- Add the pen or vial, with doses remaining and the expiry date. A refill reminder fires at three doses left.
- Log the pen rather than the dose as the vial: the 2 mg/3 ml pen gives four 0.5 mg doses or eight 0.25 mg doses, and the app counts them down.
- Log each dose when you take it, with the site. Notes for side effects go on the dose.
What to log for diabetes
- Fasting glucose, if you test. Manual entry is free.
- HbA1c every three months. On Premium, upload the lab PDF and the HbA1c line sits against the dose steps.
- Other glucose-lowering drugs. Insulin and sulfonylureas with a GLP-1 carry the low blood sugar warning. Log them so both lines are on the timeline.
- Weight, weekly.
- Eye appointments, if you have retinopathy. Rapid glucose improvement can worsen it temporarily, and the label says to keep the checks.
Ozempic for weight
If you are on Ozempic for weight without diabetes, the log is the same and the schedule tops out at 2 mg rather than Wegovy's 2.4 mg. The comparison page explains why the two labels exist for one molecule.
Logging on Peptide AI is free, and that includes GLP-1s. The dose log, the titration schedule, vials with doses remaining, injection sites with a rotation reminder, nutrition, supplements and training are all on the free tier with no card. Premium adds bloodwork, Oura, WHOOP, Garmin, Apple Health and Health Connect on the same timeline, the body scanner and Remi, the coach that answers from your own log. The pricing page has the three plans.
Questions people ask
Is there a free Ozempic tracker app?
Yes. Peptide AI logs semaglutide doses, pens, injection sites and titration free on iOS and Android. Labs and wearables on the same timeline are Premium.
