A semaglutide tracker, whichever form it arrives in
Pen, compounded vial, or tablet. The molecule is the same, the arithmetic is not, and the log has to know which one you are holding.
Semaglutide is the molecule in Ozempic, Wegovy, the Wegovy pill and Rybelsus, and it is the compound most compounding pharmacies sold during the shortage years. The pens are simple. A compounded vial arrives as a concentration in milligrams per millilitre and a syringe marked in units, and that conversion is where most logging mistakes happen. This page is for anyone on semaglutide, in any form.
The three forms
| Form | Dose | What the log needs |
|---|---|---|
| Brand pen (Ozempic, Wegovy) | 0.25 to 2.4 mg weekly | Which pen, doses left, the site |
| Compounded vial | Prescribed in mg, drawn in units | Concentration, units per dose, doses left, expiry after reconstitution |
| Tablet (Wegovy pill, Rybelsus) | 1.5 to 25 mg daily (Wegovy pill); 3 to 14 mg daily (Rybelsus) | Taken, and taken on an empty stomach |
The compounded vial, done right
A typical vial is 5 mg of semaglutide reconstituted in 2 ml of bacteriostatic water, giving 2.5 mg per ml. A 0.25 mg dose is 0.1 ml, which is 10 units on a standard U-100 insulin syringe. A 0.5 mg dose is 20 units. Get the concentration wrong by a factor of two and the dose is wrong by the same factor, which happens often enough that poison control centres have reported on it. The free semaglutide reconstitution calculator does the conversion, and the vial row in the app keeps count of doses left and the date it was mixed, because a reconstituted vial has a shelf life.
The schedules
| Period | Dose | Notes |
|---|---|---|
| Weeks 1 to 4 | 0.25 mg once weekly | Starter dose, not meant to work yet |
| Weeks 5 to 8 | 0.5 mg once weekly | |
| Weeks 9 to 12 | 1 mg once weekly | |
| Weeks 13 to 16 | 1.7 mg once weekly | |
| Week 17 on | 2.4 mg once weekly | Maintenance dose |
Compounded semaglutide is usually prescribed on the same ladder as the brand, sometimes with smaller steps. The oral ladders are on the Wegovy pill page.
Missed a dose
Ozempic: within 5 days, take it. Wegovy: take it if the next dose is more than 48 hours away. Compounded: follow whichever rule your prescriber gave, usually one of those two. Tablet: skip and take the next one tomorrow. The missed dose page has the detail.
Setting it up in Peptide AI
- Add Semaglutide from the compound library. The unit and the weekly or daily schedule are already set.
- Enter the titration your prescriber gave you. The app shows which week of which step you are on and when the next step is due.
- Add the pen or vial, with doses remaining and the expiry date. A refill reminder fires at three doses left.
- For a compounded vial, enter the milligrams and the water volume and the app gives units per dose and tracks doses left.
- Log each dose when you take it, with the site. Notes for side effects go on the dose.
How long it stays in the system
About seven days half-life, about five weeks to clear, in every form. That is why side effects follow the dose step rather than the day, and why stopping takes a month to feel. The clearance page has the comparison.
Logging on Peptide AI is free, and that includes GLP-1s. The dose log, the titration schedule, vials with doses remaining, injection sites with a rotation reminder, nutrition, supplements and training are all on the free tier with no card. Premium adds bloodwork, Oura, WHOOP, Garmin, Apple Health and Health Connect on the same timeline, the body scanner and Remi, the coach that answers from your own log. The pricing page has the three plans.
Questions people ask
How do I track compounded semaglutide?
Log the vial with its concentration and the date it was reconstituted. The app converts your prescribed milligrams to units on the syringe and counts doses remaining. The free reconstitution calculator on the site does the same maths.
How many units is 0.25 mg of semaglutide?
It depends on the concentration. At 2.5 mg per ml, which is 5 mg in 2 ml of water, 0.25 mg is 0.1 ml, or 10 units on a U-100 insulin syringe. At a different concentration the answer changes, so check the vial.
Is there a free semaglutide tracker?
Yes. Peptide AI logs semaglutide in pen, vial or tablet form free on iOS and Android, including the reconstitution maths. Labs and wearables on the same timeline are Premium.
