Foundayo, the GLP-1 pill with no food rules
Approved on 1 April 2026, taken once a day with or without food. It is not a peptide, which is why it is a tablet and why it leaves the body in a week.
Foundayo is Eli Lilly's brand name for orforglipron. It was approved by the FDA on 1 April 2026 for chronic weight management in adults with obesity, or overweight with at least one weight-related condition. It is the first GLP-1 that is a small molecule rather than a peptide, and that single fact explains most of what is different about it.
How to take it
One tablet, once a day, with or without food, at any time. Swallow it whole. That is the whole instruction, and it is the reason the drug exists. The Wegovy pill, which is a peptide, has to be taken on an empty stomach with a small sip of water and a 30 minute wait, because peptides are poorly absorbed from the gut. Orforglipron is absorbed like an ordinary tablet.
The dose ladder
| Period | Dose | Notes |
|---|---|---|
| First 30 days or more | 0.8 mg once daily | Starter dose |
| Next 30 days or more | 2.5 mg once daily | |
| Next 30 days or more | 5.5 mg once daily | |
| After that, if needed | 9, 14.5 or 17.2 mg once daily | Each step after at least 30 days |
The approved strengths are 0.8, 2.5, 5.5, 9, 14.5 and 17.2 mg. They differ from the doses in the trials, which were 6, 12 and 36 mg, so trial results do not map directly onto the approved doses. Each step is held for at least 30 days. Not everyone climbs to the top; the prescriber picks the maintenance dose on response and tolerance.
What the trials showed
In ATTAIN-1, the phase 3 trial in adults with obesity, people on the highest tested dose lost an average of around 12 percent of body weight over 72 weeks. That is less than the injectables at full dose, about 15 percent for semaglutide and about 21 percent for tirzepatide in their own trials, and the trials cannot be compared directly. What Foundayo offers is not the biggest number. It is a daily tablet with no needle, no refrigeration and no fasting, at a different price point.
Side effects
The same family as every GLP-1: nausea, vomiting, diarrhoea, constipation, reflux, mostly in the first weeks of each step. The label carries the same boxed warning on thyroid C-cell tumours seen in rodents, and the same contraindication for a personal or family history of medullary thyroid carcinoma or MEN 2. The other label warnings, pancreatitis, gallbladder disease, low blood sugar with insulin or sulfonylureas, kidney injury from dehydration, apply too. The side effects page covers them.
Missed a tablet
Skip it and take the next one at the usual time. Never two in one day. Because the half-life is short, in the range of a day or two, a missed tablet drops the level more than a missed weekly shot does, which is a reason to keep the routine fixed.
Who it suits
People who will not inject. People who travel and cannot keep a pen cold. People for whom the Wegovy pill's fasting routine does not fit the morning. People stepping down from an injectable to maintain, which prescribers are starting to do. It is less likely to suit someone chasing the largest possible loss, where the injectables still lead.
Tracking a daily pill
A daily tablet is easier to take and easier to forget. The streak is the thing to track, and so is the step date, because the 30-day holds are the calendar the prescriber works to. Peptide AI logs Foundayo as a daily compound, with the reminder, the streak on the watch, and the titration date. The Foundayo tracker page has the setup.
Questions people ask
What is Foundayo?
Eli Lilly’s brand name for orforglipron, a once-daily oral GLP-1 receptor agonist approved by the FDA on 1 April 2026 for chronic weight management. It is a small molecule rather than a peptide, so it can be taken with or without food.
What is the Foundayo dose?
Start at 0.8 mg once daily, then 2.5 mg, then 5.5 mg, each held at least 30 days. After that the dose can be raised to 9, 14.5 or 17.2 mg once daily. The maximum is 17.2 mg.
Do I have to take Foundayo on an empty stomach?
No. It can be taken with or without food, at any time of day, with water. That is the main difference from the Wegovy pill.
How much weight do people lose on Foundayo?
Around 12 percent of body weight over 72 weeks at the highest dose tested in the ATTAIN-1 trial. The approved doses differ from the trial doses, so individual results vary.
