GLP-1 drugs and fatty liver disease: the biopsy trials
In August 2025 Wegovy received accelerated approval for MASH with moderate to advanced scarring. The approval rests on liver biopsies at 72 weeks. The outcome data that would confirm it are not due for years.
Fatty liver disease has new names. MASLD, metabolic dysfunction-associated steatotic liver disease, is fat in the liver linked to weight, blood sugar or blood lipids. MASH, metabolic dysfunction-associated steatohepatitis, is the subset where the fat comes with inflammation and liver cell injury. MASH can drive fibrosis, scarring graded from F0 (none) to F4 (cirrhosis). Fibrosis stage is what predicts liver failure, liver cancer and death.
Because weight loss is known to improve the liver, the GLP-1 drugs were obvious candidates. Two trials have reported biopsy results.
The trials at a glance
| Trial | Year, journal | n | Duration | Drug and dose | Primary result |
|---|---|---|---|---|---|
| ESSENCE, part 1 | 2025, NEJM | 800 of 1,197 enrolled | 72 weeks (interim) | Semaglutide 2.4 mg weekly vs placebo | MASH resolution without worse fibrosis 62.9% vs 34.3%; fibrosis improvement without worse MASH 36.8% vs 22.4% |
| SYNERGY-NASH (phase 2) | 2024, NEJM | 190 | 52 weeks | Tirzepatide 5, 10 or 15 mg weekly vs placebo | MASH resolution without worse fibrosis 44%, 56%, 62% vs 10% |
ESSENCE: semaglutide, phase 3
ESSENCE enrolled 1,197 people with biopsy-confirmed MASH and fibrosis stage F2 or F3, moderate or advanced scarring short of cirrhosis. They were randomised two to one to semaglutide 2.4 mg or placebo, with a planned treatment period of 240 weeks. The published results are part 1: a planned interim analysis of the first 800 participants at week 72, with a repeat liver biopsy.
- Resolution of steatohepatitis without worsening fibrosis: 62.9 percent of 534 on semaglutide, 34.3 percent of 266 on placebo. Difference 28.7 percentage points.
- Fibrosis improvement without worsening steatohepatitis: 36.8 against 22.4 percent. Difference 14.4 points.
- Both at once: 32.7 against 16.1 percent.
- Body weight: down 10.5 percent against 2.0 percent.
Gastrointestinal side effects were more common on semaglutide, as in every other trial. Bodily pain scores did not differ.
Note the placebo rates. A third of people on placebo saw their MASH resolve and a fifth saw fibrosis improve. Some of that is the lifestyle advice everyone received, and some is the known variability of liver biopsy. It is why biopsy trials need a placebo group.
How a biopsy end point works
Both trials judged success by a pathologist reading liver tissue under a microscope, before and after treatment. "Resolution of steatohepatitis" means the inflammation and ballooning of liver cells that define MASH are no longer seen. "Fibrosis improvement" means the scarring stage dropped by at least one step, for example from F3 to F2. Each end point is counted only if the other measure did not get worse at the same time, so a drug cannot score by improving inflammation while scarring advances.
Biopsy is the reference standard, but it samples a tiny piece of a large organ, and two pathologists can read the same slide differently. That is one reason the placebo response is high, and one reason regulators treat these results as a surrogate rather than the final word.
The FDA approval, August 2025
The FDA granted Wegovy accelerated approval for adults with MASH and moderate to advanced liver fibrosis, F2 to F3, without cirrhosis, alongside diet and physical activity. The FDA page carries the date of 15 August 2025. Wegovy had breakthrough therapy designation for this use.
Accelerated approval matters here. It lets a drug for a serious condition reach the market on a surrogate end point, in this case biopsy findings, on condition that a confirmatory result follows. ESSENCE continues to 240 weeks to test whether the biopsy improvement translates into fewer deaths, liver transplants and other liver events. If it does not, the indication can be withdrawn.
SYNERGY-NASH: tirzepatide, phase 2
SYNERGY-NASH was a dose-finding phase 2 trial in 190 people with biopsy-confirmed MASH and F2 or F3 fibrosis. They were randomised to tirzepatide 5, 10 or 15 mg, or placebo, for 52 weeks. 157 had a usable biopsy at the end; missing values were imputed.
MASH resolved without worsening fibrosis in 10 percent on placebo, and 44, 56 and 62 percent on the three tirzepatide doses. Fibrosis improved by at least one stage without worsening MASH in 30 percent on placebo and 55, 51 and 51 percent on tirzepatide, with wide confidence intervals; the lower limits for the 10 and 15 mg arms were only 1 point above placebo.
These numbers look larger than ESSENCE, but the trials cannot be compared directly. SYNERGY-NASH was much smaller, shorter, phase 2, and its own authors concluded that larger and longer trials are needed. Tirzepatide has no MASH indication.
What this means
- The approval is narrow. MASH with F2 or F3 fibrosis, confirmed by a clinician. Simple fatty liver on an ultrasound, or cirrhosis, are outside it.
- Diagnosis is not a blood test alone. ALT and AST are often normal in MASH. Fibrosis is usually staged with a calculated score such as FIB-4, then elastography (a liver stiffness scan), and sometimes biopsy.
- Alcohol still matters. The liver benefit of weight loss can be undone by drinking. The alcohol research page covers what is known about GLP-1 drugs and drinking.
- Stopping. Weight tends to return after stopping. There is no evidence yet on whether liver gains hold.
The GLP-1 blood work page covers liver enzymes and the FIB-4 inputs, and how to read a lab report explains the reference ranges.
For the liver, the useful record is ALT, AST and platelet counts from each panel, any FIB-4 or elastography result, weekly weight and alcohol intake. The free GLP-1 tracker keeps the dose, the weight, side effects and, on Premium, lab results on one timeline, so the numbers your prescriber asks about are in one place rather than spread across portals and notes. It is free to download.
Questions people ask
Is Wegovy approved for fatty liver disease?
Wegovy received FDA accelerated approval in August 2025 for adults with MASH and moderate to advanced fibrosis (F2 to F3) without cirrhosis. It is not approved for simple fatty liver or for cirrhosis.
What did the ESSENCE trial show?
At 72 weeks, MASH resolved without worsening fibrosis in 62.9 percent on semaglutide 2.4 mg against 34.3 percent on placebo, and fibrosis improved in 36.8 against 22.4 percent. Weight fell 10.5 against 2.0 percent.
Does tirzepatide treat MASH?
In the phase 2 SYNERGY-NASH trial of 190 people, MASH resolved in 44 to 62 percent on tirzepatide against 10 percent on placebo at 52 weeks. It is a phase 2 result and tirzepatide has no MASH indication.
Why is the Wegovy MASH approval called accelerated?
It is based on biopsy findings, a surrogate end point. ESSENCE continues to 240 weeks to show whether that translates into fewer deaths, transplants and liver events.
